  • Medical information such as GP name/surgery, medical conditions, medical history and medications. (This may include sensitive information such as that relating to physical and mental health issues) 
  • Payment details (bank details, address)
  • Contact history and personal correspondence
  • Other information or feedback you may provide to us, such as in surveys

 

 

Reasons why I collect and use your personal information;

 

All collection and processing will be carried out based on adequate legal grounds, will be deemed necessary (for medical purposes/the care of the individual or to enable me, Sarah Lord Physiotherapy and Wellbeing, to comply with statutory or contractual requirements), and will require consent from the individual or (designated other/person deemed responsible for) entering into a contract with Sarah Lord Physiotherapy and Wellbeing. I also rely on your consent to send out marketing information and newsletters.

You can withdraw your consent at any time by letting me know as set out below.

I may share your information with someone who is subject to an equivalent duty of confidentiality, such as your GP or other professional, if you have given specific consent for me to do so.

From time to time, I may need to obtain information from third parties, such as your GP or other health professionals. This will only apply where it is essential for the provision of services and as permitted by law. Where necessary, applicable consent of the client or organisation providing the data will be obtained.

 

How long your personal information is kept?
I will hold your personal data for the period of your treatment and for 7 years following your last session. This is in accordance with professional (physiotherapy) guidelines.

 

Transfer of your information out of the EEA
I will not transfer your personal information to locations outside of the European Economic Area (EEA) without first informing you and ensuring that there are adequate safeguards for the protection of your privacy.

  • Access your personal information and to certain other supplementary information that this Privacy Notice is already designed to address
  • Ask me to correct any mistakes in your information which I hold

  • Object at any time to processing of personal information concerning you for direct marketing

  • Object in certain other situations to our continued processing of your personal information
  • Otherwise restrict our processing of your personal information in certain circumstances
